I have a customer that see's the following message with the NCL
show hardware all command.

Boot #1180 Reason = Line Card Failed, Device Slot = W622-4

He has some hardware-related questions I can't answer.

Does this indicate the entire DECNIS rebooted because of the error ? Or
did the line card just reload its code ?

Is the line card self test only run once during power-up, or can
it run at other times ?

If the line card fails during normal operation, what are some of the
possible indications  other than a software crash ?

this is an element from the SELF TEST ERRORS attribute of the HARDWARE entity.
It doesn't indicate that the line card caused the crash but that on self test
the line card failed. It is possible that the line card failure caused the crash
but we'd have to see a dump to say for sure.

only at power up or after a dump

depends on the failure. If it is catastrophic to the operation of the processor
on the line card then a crash would always occur. If it only affects
transmission and reception then it might be undetectable, except that the link
stops working. Some self test failures can only be detected if a loopback
connector is present.
