I have a customer that see's the following message with the NCL
show hardware all command.

Boot #1180 Reason = Line Card Failed, Device Slot = W622-4

He has some hardware-related questions I can't answer.

Does this indicate the entire DECNIS rebooted because of the error ? Or
did the line card just reload its code ?

Is the line card self test only run once during power-up, or can
it run at other times ?

If the line card fails during normal operation, what are some of the
possible indications  other than a software crash ?

If it only affects transmission and reception then it might be undetectable, except that the link stops working. Some self test failures can only be detected if a loopback connector is present.
