I have 'Modem Midi' which I bought from Club Cubase UK,
but I have never used it as I decided that I would rather
keep with 16 midi channels since I wanted to retain
'compatibility' in the midifiles I did so that they would
play back on 'standard' setups.
I believe your circuit diagram will probably be the
same circuitry that is in the 'Modem Midi' adapter (which
plugs into the modem port and gives an additional 16 channels)
- if so, the setup requirements may be the same:
With Steinberg Cubase, a version 3.xx of MROS is required,
and the ExPort driver. Install by entering the MROS folder
and ensuring that the ExPort driver appears as EXPORT.DRV
Viewing the output for each part in 'Cubase', select 'Xport 1'
to access the modem midi port.
With C-Lab Creator or Notator, the modem midi port is accessed
via. Midi Port B within the sequencer. There is also a C-Lab
utility program called 'Director' which I understand is Public
Domain, and lets you configure all Midi output to Port B. This
may be required for certain System Exclusive dumps within the
sequence.

I got the pinout diagram from the Future Music magazine. It's very
basic really.
1) Solder terminals 4 and 5 in the MIDI plug to terminals 2 and 7,
respetively, on the modem connector.
That's it! I got the connectors, wires etc in a local shop for 3.50
(UK pounds). I saw the said item in the same magazine for #12.95!!!!
