There is a template called DEC_V6.DOT for Information Mapping style documents.
Below is a modified version of the original announcement by Mike Swatko 
(who is no longer with Digital), with updated file pointers.

ANNOUNCEMENT

    The DEC Template (DEC_V6.DOT) for Word for Windows V6 or V7 is 
    available for Digital Internal Use. This is a generic Word template
    that supports both Information Mapped and non-Info. Mapped information,
    and both 7x9 and 8.5x11 page layouts.  

PURPOSE

    The DEC Template provides predefined styles and Word macro functions
    that make it easier for writers to:

    - Create user's guides, reference guides, and course documents;
    - Create either 7x9 or 8.5x11-size documents;
    - Maintain a consistent appearance, both within the document and with
      other Digital documentation;
    - Create documents using the Information Mapping methodology, although
      you can also use it for non-Info-Mapped documents;
    - Share portions of documents with other writers;
    - Convert documents to and from other formats with converters that may
      be developed in the future.

PAGE LAYOUT AND TYPOGRAPHY

    The DEC Template is designed to produce a familiar page layout that is
    aesthetically pleasing and follows Digital and SES typographical
    standards.  Note that the DEC Template will not produce headings (eg.
    block labels) that wrap in the margin.

DIALOG BOXES AND MACROS

    The DEC Template provides dialog boxes to sophisticated macros which
    make it easier to produce properly structured, consistently formatted
    documents.  These macros automate things that are normally manual
    operations in Word, such as:

    - Page headers and footers;
    - Chapter and appendix numbers;
    - Creating consistently formatted tables;
    - Inserting boilerplate text, where needed;
    - Creating a two-column formatted index.

INFORMATION MAPPING

    Through the use its styles and macros, the DEC Template supports the
    Information Mapping principles of labeling, consistency, integrated
    graphics, and accessible detail.  The DEC Template can also be used for
    documents that are not Information Mapped.

EDMS COMPLIANCE

    Using the DEC template and following the instructions in the DEC
    Template User's Guide will help to ensure that the documents you produce
    will be EDMS-compliant.

    The DEC Template User's Guide is approx 120 pages and contains numerous
    images.  Note that pages containing images might be slow to print.
