I have a Flamingo45 with:

	VMS V7.1
	Motif V1.2-4

	tc0  PMAGD      8MB ZLX-E2  BL2.1
	tc1  PMAGD-AA   2MB ZLX-E1  BL2.1
	tc2  PMAGB-BA   2MB HX      V1.1

I just installed the V4.1 kit and the server ACCVIOs if I try
to run multi-head with the HX.  The HX runs OK by itself, and
I can run dual-head with the two ZLX.

But try to run all three or one ZLX and the HX and I get the
attached crash in the server.

I renamed SYS$LIBRARY:DECW*.EXE;*, and reinstalled V7.1 and
Motif from the V7.1 SSB CDROM.  It's OK again with all three
heads.

The release notes specifically state that ZLX+HX is OK, but
it doesn't work on my system.

during installation I took all the defaults except that I said no for the
-L and -M cards.

   I have a customer with exactly the same problem.	
    
   Is there any official solution, IPMT for this problem ?

    The .0 note sayss that after a base re-install, "It's OK again 
    with all three heads."  Sounds like that was acceptable to the
    base noter.
    
    This latest note doesn't work with a base install?  And it 
    doesn't work with Open3D v4.3?  And it is also an HX+ZLX-E1+
    ZLX-E2?  And all of the heads worked together in VMS 6.2-xxx?
    
    What's new that works?  What's new that's failed?  There is
    no known problem/solution for this report.
