    I can't remember a note devoted to the various pin connections and
    suitable cables. (Mr. Moderator - if there is then move this note)
    Here's a start with the connections for Amiga 23pin to an NEC 3D
    monitor cable.
    

The Amiga to NEC 3D Cable I have is about 6 inches long and connected:

		23 pin amiga  		NEC 3D (*)
		connector              "15" pin connector

		3  -----------0------------ 1
		16 -------- shield -------- 6

		4  -----------0------------ 2
		17 -------- shield -------- 7

		5  -----------0------------ 3
		18 -------- shield -------- 8

		11 ------------------------ 13
		12 ------------------------ 14

		19 ------------------------ |6,7,8 (tied together)
		20 ------------------------ |      

    
		Pins 6,7,8 have all shields and earths tied together.		
    		(*) this is the 'native' minature 3 row, 15 pin plug on
    		the captive 3D cable.

    	This worked for me to connect a LVP16 plotter to my AMIGA 1000.
