The index of an online book I built has occasional blank lines.  The most
frequent occurrence is between an index entry and its first subentry;
however, it occurs between entries as well as subentries.

The length of the line does not appear to be a factor, as index entries
that wrap to the next line are not followed by a blank line.  It is
interesting that the blank lines show up only when scrolling one line
at a time by clicking on the stepping arrow; blank lines do not show up 
when dragging the slider to navigate the index.  These lines do not
show up in hardcopy.  

Is this a known problem?  An example follows:

This was a Bookreader bug that was fixed in FT2 Update.
You can get the entries to print by shrinking the window and expanding it 
again.

I checked to see what version of software we are using and was told that 
it is the FT2 update.  (In my original note, I incorrectly reported that
I was using BL2.)

Are we dealing with a different bug?  Or has it not yet been fixed?
