SUBJECT: US ALPHA SEED LOANER REQUEST FORM
How to Request an Alpha Seed Loaner Unit
----------------------------------------
1. Discuss the request with your CBU Alpha seed captain. If your Alpha seed
captain agrees to sponsor the request, fill out the attached Alpha seed
loaner request form.
2. Have your CBU Alpha seed captain submit the completed loaner request
form to ALPHA SEEDS @MRO. The US Alpha Seed Program Office will send
you a Loan Of Products (LOP) agreement.
3. Have your business partner sign the agreement. FAX the signed LOP back
to the US Alpha Seed Program Office at dtn 297-5640 or (508) 467-5640.
4. You will receive a scheduled shipment date from ALPHA SEEDS @MRO, usually
one week after you return a signed LOP.
Note: The US Alpha Seed Program Office orders units AFTER receiving
signed LOPs. Units usually ship 4-6 weeks after ALPHA SEEDS @MRO
receives a SIGNED LOP.
Additional Responsibilities as Alpha Seed Loaner Sponsor

1. Notify ALPHA SEEDS @MRO when any of the following information changes:
- company contact
- Digital seed unit requestor contact
- seed unit movement to a different location
2. You will receive a memo when the loan is ready to expire. The Alpha Seed
Program Office will request that you: 1) sell, 2) transfer to you local
cost center, or 3) return the seed unit.
Notify ALPHA SEEDS @MRO with your loan dispostion option.
The Alpha Seed Program Office will automatically transfer the unit to your
cost center if you do not choose a disposition option by the specified
date.
