Standard disclaimer: I'm not a lawyer nor do I play on on TV, but...
If if were me, I'd structure the document to look like other documents
that he has received from his lawyer or from the court. I'd state the
facts as simply as possible, ie "I obtain medical treatment for child
and personally take child to the doctor and make sure I stay informed
on what medical treatment child is getting", or "guide moral and social
development of the child". The judge has probably seen a virtual
parade of children in trouble with the law because of the lack or
absence of parental guidance and may well appreciate a father who
takes that role seriously.
Then I would attach any supporting documents such as the originals
of any documents or notes he has made. Make copies, give the
originals to the court, they will be as safe there as anywhere and
can be referenced later if needed. Courts tend to not like copies.

resources to fill out an outline with dates, whose, and etc.
Local HMO's and YW/Mca's also have parenting class's. Get a sheep skin
from that for the ol resume too. If there are some local community
college, with a class in physocology(sp) esp for kids, take it!!
